State survey identifies 21 tags; Witherell disputes several

CT GREENWICH — Connecticut Department of Public Health survey identifies 21 regulatory tags at Witherell nursing facility. Deputy Director of Nursing John Anantharaj reported to the June 29 board meeting that administration plans to formally dispute at least four of the findings from the five-day survey conducted June 9 to June 17. Anantharaj expressed confidence in removing at least three tags entirely and consolidating three more under a vaccination category.
The board also addressed ongoing financial challenges, with May 2026 average daily census at 158.2, down 7 percent from the prior year; however, the facility expects admissions from the soon-to-close Greenwich Woods facility to improve census figures.
